Revoking Qualified Certificates
If you wish to revoke a qualified certificate, we require formal confirmation signed with a qualified electronic signature (QES). This ensures with certainty that the revocation request originates from the authorized person and not from an unauthorized party.
Please proceed as follows:
- Download the revocation form.
- Fill in all required information completely, including:
- Serial number (can be found in Adobe Reader via: Signature Details → Certificate Details → tab Subject → field Serial Number, or using this validator Number → Serial Number).
- First name, last name, date of birth
- Nationality
- ID or passport number (as used during the original identification)
- Date and time of the last use of the certificate / last signature
- URL or name of the service used for signing (e.g., URL: https://simplesign.fidentity.ch/simplesign/ or service name: SimpleSIGN)
- Reason for revocation
- Contact information (email address, mobile phone number)
- Sign the completed form using a qualified electronic signature (QES). If you do not use your own QES provider, you can use simplesign.fidentity.ch. A one-time certificate is used for a single signature.
